Exercise is widely associated with better heart health, stronger muscles, improved metabolic health, and greater longevity. But can working out actually help prevent cancer?

The answer is more interesting than simply saying exercise is “good for you.” Research increasingly suggests that regular physical activity can reduce the risk of developing several types of cancer.

It doesn’t make anyone cancer-proof, and exercise should never replace recommended cancer screenings, medical care, avoiding tobacco, or other established prevention strategies.

Instead, physical activity appears to influence several of the biological environments in which cancer can develop—including inflammation, insulin levels, sex hormones, body composition, immune function, and gastrointestinal health.

Every workout isn’t simply burning calories. It may also be changing what is happening inside your body in ways associated with a lower long-term risk of disease.


Does Exercise Really Reduce Cancer Risk?

Yes—although scientists generally describe this relationship as reducing cancer risk, rather than guaranteeing cancer prevention.

The National Cancer Institute reports strong evidence linking higher levels of physical activity with lower risk of several cancers:

  • Breast cancer
  • Colon cancer
  • Endometrial cancer
  • Bladder cancer
  • Kidney cancer
  • Esophageal adenocarcinoma
  • Stomach cancer

The evidence isn’t equally strong for every cancer. Genetics, age, environmental exposures, tobacco, alcohol, infections, body composition, diet, and many other factors can contribute to risk.

A Study of 1.44 Million Adults

A major study published in JAMA Internal Medicine pooled data from 1.44 million adults across 12 prospective U.S. and European cohorts. Researchers examined leisure-time physical activity and the incidence of 26 different cancers.

So why might moving your body affect something as complex as cancer?

1. Exercise Can Help Control Insulin

One of the most important connections may involve insulin and metabolic health. Insulin isn’t only involved in blood sugar—it also affects cellular growth and signaling. Chronically elevated levels can create conditions that encourage cell growth and can influence insulin-like growth factor 1 (IGF-1).

Working muscles become powerful consumers of glucose. Regular activity can improve insulin sensitivity, helping your body manage glucose without requiring as much insulin.

You aren’t simply burning calories. You’re training your body to manage energy differently.

2. Exercise Can Reduce Chronic Inflammation

Inflammation isn’t inherently bad. It is part of the body’s normal response to training, injury, and infection. The problem is persistent, chronic inflammation, which can contribute to cellular damage and abnormal cell growth.

Regular activity can improve metabolic health and help regulate inflammatory processes throughout the body.

3. Exercise Can Influence Hormones

Some cancers are influenced by hormones. Estrogen exposure, for example, plays a role in certain breast and endometrial cancers.

Physical activity may help regulate sex hormones such as estrogen through its effects on body composition and metabolism. The National Cancer Institute identifies reductions in certain sex hormones and growth factors as a possible mechanism behind exercise’s association with lower cancer risk.

4. Exercise Supports a Healthy Body Composition

This shouldn’t be oversimplified into “being thinner prevents cancer.” Body fat isn’t simply stored energy. Adipose tissue is metabolically active tissue. Excess body fat can affect:

  • Inflammatory signaling
  • Insulin
  • Estrogen
  • IGF-1 and growth signals
  • Cellular growth
  • Blood vessel formation

According to the American Cancer Society, excess body weight is associated with an increased risk of multiple cancers. Exercise can regulate energy balance, preserve or build muscle, improve metabolic health, and support healthier body composition.

Exercise appears to offer benefits beyond weight loss. Being physically fit and being lighter are not necessarily the same thing.

5. Exercise May Improve Immune Function

Your immune system doesn’t only fight viruses and bacteria. It also plays an important role in recognizing and responding to abnormal cells.

The relationship between exercise, immunity, and cancer is complex, and scientists are still working to understand it. However, improved immune function is among the mechanisms the National Cancer Institute identifies as potentially contributing to reduced cancer risk.

Consistency matters more than viewing any single workout as protective.

6. Exercise May Help Protect the Colon

Physical activity can help food move through the digestive system more efficiently. The longer potentially harmful compounds remain in contact with the gastrointestinal tract, the longer tissues may be exposed to them.

  • Reduced gastrointestinal transit time
  • Altered bile-acid metabolism
  • Reduced exposure to suspected carcinogens

This may help explain why colon cancer has one of the strongest relationships with physical activity. The World Cancer Research Fund concludes there is strong evidence physical activity decreases colon cancer risk.

7. Sitting Less May Matter, Too

Going to the gym is great. But what happens during the other 23 hours of your day also matters. The World Health Organization recommends increasing physical activity and reducing sedentary behavior.

    Take short walks throughout the workday or go for a walk after dinner.

    Stand periodically and look for simple reasons to get out of your chair.

    Take the stairs, carry groceries, do yard work, play sports, and train.

Movement doesn’t only count when you’re wearing gym clothes.

Do You Need Intense Workouts?

Not necessarily. A 2025 NIH/National Cancer Institute study examined more than 85,000 adults who wore wrist accelerometers to objectively measure activity.

26%Lower risk of developing one of 13 studied cancers among people with the highest overall daily activity compared with those with the lowest activity.

Even light-intensity movement was associated with lower cancer risk. Compared with people averaging 5,000 steps per day:

7,000steps was associated with an 11% lower cancer risk
AND
9,000steps was associated with a 16% lower cancer risk
THEN
PLATEAUthe association leveled off beyond roughly 9,000 steps

This doesn’t make 9,000 steps a magical threshold.

More daily movement may matter, even when it isn’t an intense workout.

How Much Exercise Should You Get?

The American Cancer Society’s guidelines recommend:

150–300minutes of moderate activity per week
OR
75–150minutes of vigorous activity per week
PLUS
2+days of muscle-strengthening activity

The ACS notes that reaching or exceeding 300 minutes of moderate activity per week is ideal. The WHO offers similar aerobic recommendations and recommends strength training involving the major muscle groups on at least two days per week.

What Does This Look Like?

You don’t need to train like an athlete. A realistic week could combine:

  • Resistance training 2–3 days
  • Brisk walking
  • Cycling or swimming
  • Recreational sports
  • Hiking
  • Short workday walks
  • Taking the stairs
  • Increasing daily steps

If you’re currently sedentary, don’t let the recommended numbers convince you anything less is pointless. Some activity is better than none.

Is Strength Training Important?

A comprehensive fitness program should include resistance training. It helps preserve or increase skeletal muscle, maintain physical function, improve glucose metabolism, support bone health, and improve body composition.

Cardio and resistance training don’t need to compete. A well-designed fitness program can use both.

Exercise Isn’t a Force Field

A physically active person can still develop cancer. A sedentary person may never develop it. Cancer risk is affected by many variables, some modifiable and others outside our control.

The strongest evidence regarding cancer incidence still comes largely from observational research. These studies can show powerful associations, but cannot completely prove that exercise itself caused the reduction.

Fitness Is About More Than Looking Fit

Exercise is often marketed around appearance, but that represents only a fraction of what activity is doing.

  • Hormone regulation
  • Body composition
  • Immune function
  • Digestive health
  • Cardiovascular fitness
  • Muscular strength
  • Metabolic health

Collectively, those adaptations may create an internal environment associated with a lower risk of several chronic diseases—including certain cancers.


The Bottom Line

Exercise can reduce risk.

It cannot guarantee that you will never develop cancer. But the evidence strongly supports regular physical activity as one component of cancer-risk reduction.

Walk more. Sit less. Lift weights. Build muscle. Improve your cardiovascular fitness. Find activities you enjoy enough to continue doing. Most importantly, think in terms of years rather than weeks.

Fitness isn’t something you accomplish once. It’s a health behavior you continue practicing.
